He was a member of the Institute of Medicine\u2019s Committee on Quality of Health Care in America, which drafted \u201cTo Err is Human,\u201d released in 1999.<\/p>\n<p>The Institute of Medicine reports\u00a0<em>To Err is Human: Building a Safer Health System<\/em>, published 20 years ago, followed by\u00a0<em>Crossing the Quality Chasm: The IOM Health Care Quality Initiative<\/em>\u00a0two years later, have led to considerable efforts to improve healthcare.
